Orthodontic Emergencies: What’s Urgent and What Isn’t?

When you or your child is in braces or Invisalign, it’s normal to wonder: “Is this an orthodontic emergency, or can it wait?”

What Counts as a True Orthodontic Emergency?

Most issues with braces and aligners are uncomfortable or inconvenient—but not dangerous. A true emergency is usually related to:

  • Severe pain that does not improve with over-the-counter pain relievers.
  • Heavy bleeding that doesn’t stop.
  • Facial or oral trauma (like from a fall or sports injury).
  • Suspicion that part of an appliance has been inhaled or is blocking breathing.

In these situations:

  • Seek emergency medical or dental care first (urgent care or ER) if breathing, bleeding or major trauma is involved.
  • Once you are safe and stable, contact Othman Orthodontics so we can assess and repair your braces or appliances.

Common Orthodontic Problems That Are Not True Emergencies

These issues are very common and usually not dangerous, but they can still be uncomfortable:

Poking or Long Wire

As teeth move, wires can shift and feel longer, irritating cheeks or gums. This is one of the most common complaints with braces.

Loose Bracket

A bracket may come loose after biting something hard or sticky, or if it’s bumped. Often, it remains attached to the wire and “slides” slightly.

Loose Band or Appliance

The metal band around a back tooth, or part of an appliance (like an expander), can loosen but usually remains in place.

Soreness After Adjustments or New Aligners

After braces adjustments or when starting a new set of Invisalign aligners, mild soreness or pressure is very normal.

Irritation from Brackets, Wires or Aligners

Brackets and wires can rub on the cheeks or lips, especially at the start of treatment. Invisalign edges can sometimes feel “sharp” until your mouth adjusts.

Lost or Cracked Clear Aligner

Misplacing or cracking a clear aligner can happen, especially with busy teens and adults.

While these problems aren’t usually emergencies, we still want to know about them—so we can help you stay comfortable and keep treatment on track.

How to Handle Common Problems at Home

1. Poking Wire

If a wire is poking:

  • Dry the area and place orthodontic wax over the end of the wire.
  • If wax is not available, a small piece of clean cotton can help temporarily.
  • If you feel confident, you may carefully trim the very end of the wire with clean nail clippers, then call our office.

Always let us know if a wire is constantly poking, so we can adjust it properly at the office.

2. Loose Bracket

If a bracket is loose but still attached to the wire:

  • Gently slide it back into a comfortable position.
  • Use orthodontic wax if it’s irritating your cheek or lip.
  • Call us to see whether we need to see you sooner or can fix it at your next visit.

If the bracket has come completely off:

  • Save it in a small container or bag.
  • Bring it with you to your appointment.

3. Loose Band or Expander

If a band or appliance feels loose:

  • Do not try to remove it yourself.
  • Use wax to cover any rough or irritating edges.
  • Contact our office so we can evaluate and repair it.

4. General Soreness

Mild soreness after starting braces or Invisalign—or after adjustments—is common. To help:

  • Eat soft foods like yogurt, pasta, mashed potatoes or smoothies.
  • Rinse with warm salt water.
  • Use over-the-counter pain relievers as recommended by your physician.

This soreness usually improves within a few days and is not an emergency.

5. Irritation of Cheeks or Lips

If brackets or wires rub on your cheeks:

  • Use orthodontic wax over the irritating area.
  • Give your mouth time to adjust—soft tissues typically “toughen up” after a week or two.
  • Call us if sores are severe or not improving.

6. Lost or Cracked Clear Aligner

If you’re in Invisalign or clear aligner treatment and you lose or break an aligner:

  • If you are near the end of that aligner’s wear time, move to the next aligner and call us.
  • If you just started that aligner, go back to the previous one and call our office for instructions.
  • Do not skip ahead more than one aligner without our guidance.

When You Should Call Us Right Away

Call our Lombard or Orland Park office as soon as possible if:

  • You are experiencing significant pain that is not improving with home care.
  • You had a sports injury or fall that affected your mouth, teeth or braces.
  • A part of your appliance feels like it could be swallowed or is moving toward your throat.
  • Your aligners or braces feel so uncomfortable that you cannot wear them.

How to Prevent Orthodontic “Emergencies”

While some accidents are unavoidable, you can reduce your risk of problems by:

  • Avoiding sticky and hard foods (like caramel, taffy, nuts, ice and popcorn kernels).
  • Wearing a mouthguard for contact sports or activities where you might be hit in the face.
  • Not biting pens, fingernails or other hard objects.
