A loose bracket, poking wire or sore tooth can be stressful—especially if you’re not sure whether it’s a true emergency.

What Is a True Orthodontic Emergency?
Orthodontic “emergencies” are usually not life-threatening. However, there are situations that require immediate attention—either from our office or, rarely, from a medical provider.
Call a Medical Provider or ER If You Have:
- Significant facial trauma from an accident or sports injury.
- Heavy, uncontrolled bleeding in the mouth or face.
- Suspected broken jaw or severe facial swelling.
- Difficulty breathing or swallowing.
Once urgent medical needs are addressed, we can evaluate your orthodontic appliances and coordinate repairs.
Call Our Office Promptly If You Have:
- A wire or appliance that is causing persistent pain or cutting the cheeks, lips or tongue.
- A broken or loose appliance (like a palatal expander) that can’t be stabilized.
- A bracket or band that has come loose and is sliding around the wire.
- Severe pain that doesn’t improve with over-the-counter pain relievers (as approved by your doctor).
These aren’t usually medical emergencies, but they do need timely orthodontic attention to keep you safe and on track.
Common Minor Issues & How to Handle Them at Home
1. Poking Wire
Sometimes, as teeth move, the wire can shift and poke the back of the mouth.
You can:
- Dry the area and place orthodontic wax over the poking end.
- If instructed by our office, use a clean nail clipper or small cutter to trim a very small amount of the extra wire.
- Call us if the wire is causing ongoing pain or keeps poking despite wax.
2. Irritated Cheeks or Lips
It’s common to have sore spots when braces are new or after adjustments.
- Rinse with warm salt water to soothe irritated tissues.
- Use orthodontic wax on brackets or areas rubbing against cheeks or lips.
- Over time, your mouth will toughen and irritation usually decreases.
3. Loose Bracket
A bracket can come loose if something hard or sticky is eaten, or from trauma.
If the bracket is:
- Still on the wire: Gently slide it back toward its normal position and cover with wax if needed.
- Completely off: Save it and bring it to your next appointment.
Call our office and we’ll let you know whether it needs to be fixed right away or can wait until your next scheduled visit.
4. Loose Band or Expander Part
If a metal band or part of an appliance like an expander feels loose:
- Stop turning the expander if you’re in an active expansion phase and call us.
- Avoid sticky or hard foods that might worsen the looseness.
- We will schedule a repair as soon as possible.
5. Soreness After Adjustments
It’s normal to feel:
- Pressure or mild soreness for a day or two after braces are tightened or new aligners are started.
To manage:
- Use over-the-counter pain relievers as recommended by your physician.
- Eat softer foods for a day or two.
- Rinse with warm salt water if gums or soft tissue feels irritated.
6. Lost Clear Aligner or Retainer
If you lose an Invisalign aligner or retainer:
- Call us to see if you should move to the next aligner, go back to the previous one or have a replacement made.
- Don’t simply stop wearing aligners—teeth can start shifting surprisingly quickly.
What’s Not an Emergency (But Still Important)
The following are usually not emergencies, but you should still contact us for guidance:
- Broken elastic chain or missing colored elastics.
- Minor discomfort from moving teeth.
- Questions about how much soreness is “normal.”

Preventing Orthodontic Emergencies
A few habits go a long way:
- Follow the braces diet guidelines we provide.
- Avoid chewing on ice, pens or fingernails.
- Wear a mouthguard during sports and active play.
- Handle aligners and retainers carefully and keep them in a case.
