A poking wire, a loose bracket or a broken retainer can be stressful—especially if it happens at night or on the weekend. The good news? Most orthodontic emergencies are not true emergencies and can be temporarily managed at home until you can visit our office.

What Is a True Orthodontic Emergency?
A true emergency usually involves:
- Severe pain that you cannot manage with simple remedies.
- Trauma to the face, mouth or teeth (like a sports injury or accident).
- Significant swelling, bleeding or signs of infection.
- A situation that makes it hard to breathe or swallow.
If you or your child has:
- Heavy bleeding that doesn’t stop
- Suspected broken jaw
- Difficulty breathing
go to the nearest emergency room or call emergency services first. Then contact us to coordinate follow-up orthodontic care.
Most other issues—like broken brackets, poking wires or sore teeth—are uncomfortable but not dangerous and can be handled with simple steps until we see you.
Common Orthodontic Problems & Home Fixes
1. Soreness After Braces Are Placed or Adjusted
It’s normal to feel:
- Soreness or pressure in the teeth for a few days.
- Minor tenderness in the cheeks or tongue as they get used to braces.
What You Can Do at Home
- Stick to soft foods (yogurt, mashed potatoes, pasta, smoothies) for a few days.
- Use over-the-counter pain relievers as recommended by your physician (and appropriate for your child’s age).
- Rinse with warm salt water to soothe sore areas (½ teaspoon of salt in a glass of warm water).
This type of discomfort is not an emergency and usually improves within 3–5 days.
2. Poking Wire
Sometimes a wire can shift or extend out of the last bracket, causing irritation to the cheek or gums.
Home Fixes
- Dry the area and place a small ball of orthodontic wax (provided at your visits) over the end of the wire.
- If wax is not available, sugar-free gum can work temporarily.
- If the wire is very long and you feel comfortable, you may use clean nail clippers or a cuticle trimmer to carefully snip the end of the wire—only if it’s safe and accessible.

3. Loose Bracket
A bracket can come loose from the tooth if:
- You bite into something too hard or sticky.
- You experience trauma to the mouth.
What to Do
- If the bracket is still attached to the wire, leave it in place and cover it with wax if it’s irritating.
- If it comes completely off, store it in a small container or plastic bag and bring it with you to your appointment.
A loose bracket is usually not urgent, but you should contact us soon so we can rebond it and keep your treatment on track.
4. Loose Band or Appliance
If a metal band around a molar or a part of an appliance (like an expander or habit appliance) becomes loose:
- Do not try to remove it on your own.
- Place wax over any sharp edges.
- Call our office to schedule a repair appointment.
If a piece is completely loose and removable, place it in a case or bag and bring it with you.
5. Irritated Cheeks, Lips or Tongue
When braces or appliances are new, soft tissues in your mouth may become irritated where they contact brackets or wires.
Home Care Tips
- Use orthodontic wax over any brackets or areas that rub.
- Rinse with warm salt water to promote healing.
- Use a small amount of over-the-counter oral anesthetic gel on particularly sore spots if advised by your dentist or physician.
This usually improves as your mouth adapts. If irritation persists or worsens, call us for an evaluation.
6. Broken or Lost Retainer
Retainers are essential for keeping your teeth in their new positions after braces or Invisalign. If a retainer:
- Cracks or breaks
- Feels significantly warped or doesn’t fit
- Is lost (especially clear retainers)

What You Can Do
- Keep wearing the retainer only if it fits comfortably and is not cracked in a way that could cut soft tissues.
- If it’s broken or painful, stop using it and call our office.
We may need to take new scans or impressions to make a replacement retainer and prevent shifting.
7. Food Caught in Braces
Food stuck in braces is not an emergency, but it can be uncomfortable and embarrassing.
Quick Fixes
- Try brushing and rinsing several times.
- Use interdental brushes or floss threaders to get between brackets.
- A water flosser can help dislodge stubborn particles.
