What Counts as an Orthodontic Emergency?

Most problems with braces and aligners are not true dental emergencies, but some require prompt attention. In general, call us as soon as possible if you experience:

  • Severe pain that does not improve with over-the-counter pain relievers.
  • Trauma to the face or mouth (such as a fall or sports injury).
  • Broken teeth or significant bleeding.
  • A large appliance that has become loose or is at risk of being swallowed.

For trauma or suspected broken bones, always seek urgent or emergency medical care first. We can then coordinate follow-up orthodontic care as needed.

Common Orthodontic Problems & How to Handle Them

1. Poking Wire

A wire that has shifted or become long can irritate the cheek or gums. To manage this temporarily:

  • Dry the area and place orthodontic wax over the sharp end.
  • If you cannot get wax to stay, you may use a small piece of clean cotton as a temporary cushion.
  • If the wire is very long and you are comfortable doing so, you may carefully clip it with sterilized nail clippers, then call our office to schedule a visit.

Always let us know about persistent poking wires so we can adjust them properly.

2. Loose or Broken Bracket

A bracket can come loose from the tooth, usually after biting something hard or sticky. If this happens:

  • If the bracket is still attached to the wire, leave it in place and cover it with wax if it’s irritating.
  • If it has come completely off, place it in a small container or bag and bring it to your next appointment.
  • Call our Lombard office to let us know so we can decide whether to see you sooner.

A loose bracket is usually not an emergency, but we want to fix it to keep your treatment on track.

3. Loose Band or Appliance

If a metal band around a molar or part of an expander or other appliance becomes loose:

  • Do not try to pull the appliance off yourself.
  • Call us as soon as you can so we can re-cement or repair it.
  • If it’s irritating, use wax to cover any rough spots until your appointment.

4. Soreness After Adjustments

It’s common to feel some soreness or pressure after braces are placed or adjusted, or when starting a new set of aligners. To relieve discomfort:

  • Eat soft foods for a few days.
  • Rinse with warm salt water.
  • Use over-the-counter pain relievers as recommended by your physician or pharmacist.

This soreness usually improves within a few days and is not an emergency.

5. Irritation from Brackets or Aligners

New braces or aligners can rub on cheeks, lips or tongue. To help:

  • Use orthodontic wax over brackets or areas causing irritation.
  • Give your mouth a little time to adapt—soft tissues usually “toughen up” over a week or two.
  • Call us if sores are severe or not improving.

6. Lost or Cracked Clear Aligner

If you are in Invisalign or clear aligner treatment and an aligner is lost or cracked:

  • If you were close to the end of the current aligner’s wear time, move to the next aligner and call us.
  • If you just started a new aligner, go back to the previous one and call our office.
  • Never skip ahead more than one aligner without checking with us.

We’ll help you decide whether we need to order a replacement or adjust your wear schedule.

7. Swallowed Appliance Piece

If you think you or your child may have swallowed a piece of an appliance and you are having trouble breathing, call emergency services or go to the emergency room immediately. Once you are safe, contact our office so we can assess and repair your orthodontic appliances.

While not everything can be prevented, you can reduce the risk of problems by:

  • Avoiding sticky and hard foods that can break brackets or bend wires.
  • Wearing a mouthguard for sports and physical activities.
  • Following our instructions about how to care for braces and aligners.
  • Keeping regular appointments so we can monitor your progress and appliances.
